Frequently Asked Questions: First Office Visits

All new patients are required to complete our initial intake form. Please bring in the completed form or arrive 15-minutes early if you need to complete it at the clinic.
Bring in all of your medications and nutritional supplements so they can be properly reviewed. If you are currently taking a large number of medications or supplements, please write out a list with the name the dosage in addition to bringing in the medicines and supplements.
Bring in the name and contact information for your primary care provider for records release requests and collaborations if and when needed.
*Review information on our Health Investment Program (HIP). Established patients can enroll in this annual program desined to reduce the expeses associated with some of our specialty services and frequent office visits.
